If you have been reading about intranasal delivery and want a single page that covers the useful parts, this is it: definitions, context, how it is studied, and the questions that come up repeatedly.
Last reviewed on 2026-04-18. Where a claim depends on a specific study, the study is described rather than over-claimed.
Published studies examine a fairly narrow set of endpoints. Rodent experiments commonly measure maze learning, infarct volume after induced ischemia, and tissue levels of neurotrophic factors. Clinical reports from Russian centres describe attention, memory and recovery scores in patients after stroke or transient ischemic attack. Most of those human studies are small and few have been repeated by independent groups. Outcome measures differ between studies, which limits direct comparison.
Circulation time for the peptide is short because peptidases cleave it readily. The Pro-Gly-Pro tail is thought to slow breakdown compared with the bare ACTH fragment, but the gain appears modest. Absorption after intranasal dosing is limited, and only a fraction of a dose is expected to reach the central nervous system. Laboratory concentrations therefore sit well above levels achieved systemically, a gap that complicates translation from bench findings to clinical claims.

Published research covers ischemic stroke, traumatic brain injury, cognitive impairment, optic nerve conditions and attention-related measures. Much of the human evidence comes from small trials conducted in one country, which limits how far the results generalize. Animal models supply the larger share of the data, and effects seen in rodents do not transfer automatically to people. Reviews have noted that methodological reporting is often incomplete, making it difficult to pool results or compare treatment schedules across studies.
Pharmacokinetic accounts emphasize rapid breakdown. After intravenous dosing the intact peptide disappears from blood within minutes, and nasal delivery produces low but measurable concentrations. Metabolites rather than the parent molecule may account for part of the observed activity, although the relative contribution is unresolved. Dosing in the literature varies widely and no optimal schedule has been agreed. These gaps are regularly cited as a reason the findings have not produced broad clinical adoption beyond the original research setting.
Proposed mechanisms center on neurotrophic signaling rather than on classical melanocortin receptor activation. Rodent experiments have reported shifts in the expression of brain-derived neurotrophic factor and nerve growth factor after administration, together with changes in the associated receptor systems. Several authors argue that the peptide acts largely through its degradation products and their interaction with peptidergic pathways, but this remains a hypothesis rather than a settled finding. No single molecular target has been identified in a way that the field broadly accepts.
Identity and purity of Semax are established mainly by reversed-phase high-performance liquid chromatography coupled with mass spectrometry. The chromatographic trace gives a purity estimate as a percentage of total peak area, while electrospray or matrix-assisted laser desorption ionization confirms the molecular mass against the calculated value. Amino acid analysis and sequence-specific fragmentation provide further confirmation when a supplier's chain of custody is unclear. Vendors frequently quote a purity figure without stating the detection wavelength or the integration method, which limits how far one number can be compared with another.
Stability depends heavily on physical state. Lyophilized powder held dry, cold and dark retains its content over long periods, whereas dissolved peptide begins to change within days at room temperature. The most cited degradation route is oxidation of the methionine residue, which converts the peptide to a sulfoxide form that elutes differently on chromatography. Hydrolysis of amide bonds and adsorption onto container walls contribute smaller losses. Buffers that exclude oxygen from the headspace slow the oxidation pathway, but no single condition prevents all change indefinitely.
Practical handling follows from those properties. Bulk material is best divided into single-use portions soon after receipt, because each thaw exposes the whole container to moisture and temperature cycling. Vials should be allowed to reach room temperature before opening to prevent condensation on the powder. Low-binding plasticware reduces loss of dilute solutions, and sterile filtration is used when a preparation must remain free of microbial growth. Records of batch number, reconstitution date and storage history are what allow a later analytical result to be interpreted meaningfully.
First the image is drawn out. Second asterisks are placed by potential stereocenters as indicators (*). Third the pairs of each stereocenters R and S orientations are combined through all possible permutations of stereocenters. Fourth the permutations are checked for super-imposability, indicating that they are varying around what is not a true stereocenter and therefore the permutations are truly the self-same stereoisomer. This procedure will be used when making an assessment of the number of valid stereoisomers that an analogue of fentanyl will be predicted to have. Because the analogues of fentanyl are so large, and the moieities (sub-regions) of the molecule that are relevant to its chirality are so small, that we often reproduce the fentanyl analogue as a smaller, simpler molecule with the same number of, and dynamics between its, stereocenters. This saves significant space allowing us not to reproduce redundant material that consumes a lot of space on the image. The first case studied here is fentanyl itself, or propionyl-4-anilino-N-phenethylpiperidine, the most well known of the fentanyl analogues and the eponymous molecule (namesake) for the whole chemical category. The only identified stereocenter is found at the 4-carbon, opposite the piperidine nitrogen but adjacent to the aniline nitrogen. This seems to be a stereocenter because of the apparent 4 unique substituents. We transfer this stereocenter to the equivalent molecule cyclohexanol, with an analogous apparent stereocenter.
=== America, Iran, and Under Milk Wood, 1950–1953 === American poet John Brinnin invited Thomas to New York, where in February 1950 they embarked on a lucrative three-month tour of arts centres and campuses. The tour, which began in front of an audience of a thousand at the Kaufmann Auditorium of the Poetry Centre in New York, took in about 40 venues. During the tour, Thomas was invited to many parties and functions and on several occasions became drunk—going out of his way to shock people—and was a difficult guest. Thomas drank before some of his readings, though it is argued he may have pretended to be more affected by it than he actually was. The writer Elizabeth Hardwick recalled how intoxicating a performer he was and how the tension would build before a performance: "Would he arrive only to break down on the stage? Would some dismaying scene take place at the faculty party? Would he be offensive, violent, obscene?" Caitlin said in her memoir, "Nobody ever needed encouragement less, and he was drowned in it." On returning to Britain, Thomas began work on two further poems, "In the white giant's thigh", which he read on the Third Programme in September 1950, and the incomplete "In country heaven". In October, Thomas sent a draft of the first 39 pages of 'The Town That Was Mad' to the BBC. The task of seeing this work through to production as Under Milk Wood was assigned to the BBC's Douglas Cleverdon, who had been responsible for casting Thomas in 'Paradise Lost'.
Ubayd Allah ibn Ziyad (Arabic: عُبَيْدِ اللَّهِ بْنِ زِيَادٍ, romanized: ʿUbayd Allāh ibn Ziyād) was the Umayyad governor of Basra, Kufa and Khurasan during the reigns of caliphs Mu'awiya I (r. 661–680) and Yazid I (r. 680–683), and the leading general of the Umayyad army under caliphs Marwan I (r. 684–685) and Abd al-Malik (r. 685–705). He virtually inherited the governorships from his father Ziyad ibn Abihi after the latter's death in 673. During Ubayd Allah's governorship, he suppressed Kharijite and Alid revolts. In the ensuing Battle of Karbala in 680, Husayn ibn Ali and his small retinue were slain by Ubayd Allah's troops, shocking many in the Muslim community. Ubayd Allah is primarily remembered for his role in the killings of members of Ali ibn Abi Talib's family and he has become infamous in Muslim tradition. Ubayd Allah was ultimately evicted from Iraq by the Arab tribal nobility amid the revolt of Abd Allah ibn al-Zubayr. He made it to Syria where he persuaded Marwan I to seek the caliphate and helped galvanize support for the flailing Umayyads. Afterward, he fought at the Battle of Marj Rahit in 684 against pro-Zubayrid tribes and helped reconstitute the Umayyad army. With this army he struggled against rebel Qaysi tribes in the Jazira before advancing against the Alids and Zubayrids of Iraq. However, he was slain and his forces routed at the Battle of Khazir by Ibrahim ibn al-Ashtar, the commander of the pro-Alid Mukhtar al-Thaqafi of Kufa.

The clinical course is determined by an immunological interplay between two types of T cells (Th1 and Th2 responses). SCD is often considered a subset of SNAS, but with only skin manifestations. SNAS presents with an array of symptoms ranging from respiratory to generalized skin rash to gastrointestinal symptoms. The gastrointestinal symptoms may mimic those of irritable bowel syndrome. A meta review evaluating SNAS found that 1% of patients sensitized to nickel reacted to the nickel content of a 'normal' diet, and with increasing doses of nickel more individuals reacted. SNAS is a multilayered immunological response demonstrating variance between individuals and doses of nickel exposure.
Strychnine is very toxic to humans (minimum lethal oral dose in adults is 30–120 mg) and many other animals (oral LD50 = 16 mg/kg in rats, 2 mg/kg in mice), and poisoning by inhalation, swallowing, or absorption through eyes or mouth can be fatal. S. nux-vomica seeds are generally effective as a poison only when they are crushed or chewed before swallowing because the pericarp is quite hard and indigestible; poisoning symptoms may therefore not appear if the seeds are ingested whole.
=== Market response === Several U.S. food manufacturers publicly stated that they did not use the product in their wares, including ConAgra Foods Inc., Sara Lee Corporation and Kraft Foods Inc. Many meat retailers stated that they either did not use the product, or would cease using it.
Diphenhydramine is not typically used to treat anxiety because its long-term use may cause adverse effects, such as memory loss, especially in the elderly. Diphenhydramine is not approved by the US Food and Drug Administration (FDA) for treating anxiety. On the other hand, hydroxyzine, a first-generation antihistamine that lacks significant anticholinergic effects, may be used to treat anxiety, although benzodiazepines and antidepressants are considered more effective by most clinicians. The mild anxiolytic effects of hydroxyzine are mostly due to its weak but significant activity as an antagonist of the 5-HT2A receptor, a common target of most antidepressant drugs (as well as certain other antihistamines like cyproheptadine and promethazine). Diphenhydramine is not known to bind to the 5-HT2A receptor, though it is a weak antagonist of the related 5-HT2C receptor, which is another target of antidepressant drugs and has a significant role in mood and anxiety.

=== Kinases === Posttranslational modification of proteins with phosphate groups by kinases is a key regulatory step throughout all biological systems. Phosphorylation events, either phosphorylation by protein kinases or dephosphorylation by phosphatases, result in protein activation or deactivation. These events have an impact on the regulation of physiological pathways, which makes the ability to dissect and study these pathways integral to understanding the details of cellular processes. There exist a number of challenges—namely the sheer size of the phosphoproteome, the fleeting nature of phosphorylation events and related physical limitations of classical biological and biochemical techniques—that have limited the advancement of knowledge in this area. Through the use of small molecule modulators of protein kinases, chemical biologists have gained a better understanding of the effects of protein phosphorylation. For example, nonselective and selective kinase inhibitors, such as a class of pyridinylimidazole compounds are potent inhibitors useful in the dissection of MAP kinase signaling pathways. These pyridinylimidazole compounds function by targeting the ATP binding pocket. Although this approach, as well as related approaches, with slight modifications, has proven effective in a number of cases, these compounds lack adequate specificity for more general applications. Another class of compounds, mechanism-based inhibitors, combines knowledge of the kinase enzymology with previously utilized inhibition motifs.
The first fully synthetic opioid was meperidine (Demerol), found serendipitously by German chemist Otto Eisleb (or Eislib) at IG Farben in 1932. Meperidine was the first opioid to have a structure unrelated to morphine, but with opioid-like properties. Its analgesic effects were discovered by Otto Schaumann in 1939. Gustav Ehrhart and Max Bockmühl, also at IG Farben, built on the work of Eisleb and Schaumann. They developed "Hoechst 10820" (later methadone) around 1937. In 1959 the Belgian physician Paul Janssen developed fentanyl, a synthetic opioid with 30 to 50 times the potency of heroin. Nearly 150 synthetic opioids are now known.

It consists mainly of animal experiments and small clinical reports, with much of the clinical material published in Russian-language journals. Large independent trials are scarce. Separating reliable effects from chance findings is consequently difficult.
A fraction of an intranasal dose is thought to reach the central nervous system, and this route is favoured partly for that reason. The proportion is not well characterized. Direct measurement in humans remains difficult with current methods.
Raised BDNF and NGF expression has been reported across several animal studies, which makes the signal fairly consistent. Whether the same change occurs in humans at usable doses is not established. It is best treated as a working hypothesis rather than a confirmed clinical mechanism.
